In the open discussion on women's health, @knoxknox proposed a randomized trial of traditional Finnish sauna in perimenopausal women. That is option 2 from the P16 shortlist made concrete. So this post does what P16 promised: it publishes option 2 as a comparable one-pager.
Option 2: Perimenopause sauna pilot
Primary question. Is a standardized Finnish sauna protocol feasible, tolerable and safe for perimenopausal women over 12 weeks, and how large are its early effects on blood pressure, insulin sensitivity and hot flashes?
Target cohort. 40 to 60 women in perimenopause by STRAW+10 staging. Excluded: pregnancy, uncontrolled hypertension, recent cardiovascular events, and other contraindications to heat exposure.
Minimum protocol metadata. The session standard set out earlier: modality, temperature, humidity, round duration and count, cooling method, timing, and acute symptoms. Added for this cohort: cycle day where cycles persist, and hormone therapy use.
Primary endpoints. Recruitment rate, adherence, dropout, adverse events.
Secondary endpoints. Ambulatory blood pressure, insulin sensitivity, hot flash frequency and severity, sleep quality. Exploratory: estradiol, FSH, cortisol, heat shock proteins, and core temperature and HRV during sessions.
Safety and stop rules. Supervised first sessions. Progression toward 80 to 95°C, 15 to 20 minutes, four times per week, only after symptom review. Stop for fainting, chest pain, severe headache, sustained palpitations, or prolonged next-day symptoms.
Phase-one scope. One site with sauna access, one academic partner, ethics approval, and 12 weeks of intervention against a control arm.
What counts as success. At least 70% adherence, no serious heat-related adverse events, and effect estimates precise enough to size a fully powered trial.
The open challenge
A one-pager is not a protocol. To turn this into a study plan, Steam Collective is opening a formal challenge: Design the perimenopause sauna pilot. Contributions are reviewed in public, credited to their authors, and become the plan that goes on the ballot.
What happens next
The other P16 options stay open for critique. When option 2 has a protocol, it goes to the vote alongside them with the same fields filled in. The full trial comes after, sized from what the pilot measures.
Heat. Cold. Community. Science.
