Anti-IL-17 Agents Capture 37% of Biologic Prescriptions in Spondyloarthritis: Accelerated Adoption in a Mexican Public Healthcare Cohort of 102 Patients

Background
In spondyloarthritis (SpA) and psoriatic arthritis (PsA), anti-TNF agents remain the standard first-line biologic worldwide. Anti-IL-17 agents (secukinumab, ixekizumab) were approved more recently and their uptake varies widely. European registries report anti-IL-17 accounting for 10-20% of biologic prescriptions in SpA/PsA as of 2024.
Hypothesis
In Mexican public healthcare, anti-IL-17 adoption in SpA/PsA has outpaced international registries, reaching 37% of biologic prescriptions — driven by documented anti-TNF failures and formulary dynamics. This accelerated transition, combined with population-specific genetic factors (low HLA-B27 prevalence in Maya populations, distinct IL-23 pathway genetics), may generate different efficacy profiles than predicted by European and North American clinical trials.
Preliminary Data
102 consecutive SpA/PsA patients at a tertiary IMSS hospital in southeastern Mexico (Maya-mestizo population).
Demographics
- 102 unique patients
- Women: 65 (64%) | Men: 33 (32%)
- Mean age: 53.1 years (range 23-84)
- Active outpatient: 74 (73%) | Pensioned: 23 (23%) | Student: 1
Diagnoses
- Psoriatic arthritis: 64 (63%) — dominant diagnosis
- Axial spondyloarthritis: 21+ patients
- Inflammatory bowel disease + SpA: 4 (CUCI 3, Crohn 1)
- Espondilitis anquilosante overlap throughout
Critical Finding: Biologic Distribution by Mechanism
| Mechanism | n | % | Agents |
|---|---|---|---|
| Anti-TNF | 37 | 59% | Etanercept 12, Adalimumab 9, Golimumab 8, Certolizumab 7, Infliximab 1 |
| Anti-IL-17 | 23 | 37% | Secukinumab 16, Ixekizumab 7 |
| JAK inhibitors | 3 | 5% | Tofacitinib 1, Upadacitinib 1, Baricitinib 1 |
Anti-IL-17 at 37% is approximately double the proportion reported in most European SpA registries (15-20%).
Treatment Sequencing and Failures
10 documented treatment failures/switches reveal active sequencing:
- Anti-TNF to anti-IL-17 switches: multiple (etanercept inefficacy to secukinumab, certolizumab failure to ixekizumab, infliximab failure to ixekizumab)
- Drug supply interruptions: 2 patients (certolizumab, secukinumab desabasto)
- Adverse event: 1 urticaria on etanercept
- Anti-IL-17 failure: 1 patient (secukinumab failure documented)
- TB latent: 1 patient (precluding anti-TNF, redirected to other agents)
Extra-Articular Manifestations
- Anterior uveitis: 3 patients (all on anti-TNF or anti-IL-17)
- Proctitis: 1 (secukinumab — notable given IL-17 inhibition concerns in IBD)
- Palmoplantar psoriasis: 1
- Urolithiasis: 1
Context: Contrasting Treatment Paradigms Across Autoimmune Diseases
This cohort is part of a larger 607-patient dataset from the same institution revealing disease-specific treatment dominance patterns:
| Disease Category | n | Dominant Biologic | Ratio |
|---|---|---|---|
| RA/SLE/general autoimmune | 321 | Rituximab | RTX:anti-TNF = 2.6:1 |
| Autoimmune ILD | 95 | Tacrolimus | TAC:MMF = 1.5:1 |
| APS/overlap | 89 | Rituximab | RTX dominant |
| SpA/PsA | 102 | Anti-TNF | TNF:IL-17 = 1.6:1 |
Each autoimmune disease category follows a distinct treatment paradigm — some aligning with guidelines (SpA), others diverging dramatically (RA, ILD). This suggests clinicians are making nuanced, disease-specific decisions rather than applying formulary constraints uniformly.
Genetic Considerations
- HLA-B27 prevalence in Maya populations is low (1-3% vs 6-9% European) — yet SpA clearly exists in this population, implying non-HLA-B27 genetic drivers
- IL-23R protective variant (rs11209026) shows massive signals in FinnGen SpA analysis (p=5.55x10-9) but frequency in Maya populations is unknown
- The rapid anti-IL-17 adoption may be pharmacogenomically favorable if IL-17 pathway genetics differ in this population — our FinnGen cross-reference shows all 24 IL-17 pathway genes lack published Mexican frequencies
Testable Predictions
- Anti-IL-17 response rates in Maya-mestizo SpA/PsA will exceed European trial rates (predicted by distinct IL-17 pathway genetics)
- HLA-B27 negative axial SpA prevalence will be higher in this cohort than European registries
- The anti-TNF to anti-IL-17 switch success rate will exceed 70%, supporting early IL-17 positioning in this population
- IBD-associated SpA patients on anti-IL-17 will show higher GI adverse event rates, consistent with IL-17 gut biology
Significance
This is the first report documenting anti-IL-17 adoption rates in a Mexican public healthcare SpA/PsA cohort. The 37% figure suggests that real-world clinical practice in LMICs may be more advanced in biologic sequencing than registries suggest, driven by accumulated experience with anti-TNF failures. Combined with our 607-patient multi-disease dataset, this reveals a sophisticated, disease-specific prescribing landscape in an underrepresented population.
Limitations
- Single-center retrospective data
- No standardized disease activity scores (BASDAI, DAPSA)
- Treatment duration and response not captured
- Some patients lack biologic assignment data
- Supply interruptions may confound treatment selection
